- A document that is perfectly straight
- A good-quality photo (not blurry)




The service provider’s logo

The registration number and/or first name and surname

The date the service was performed

The type of service provided

The amount billed

Proof of payment

It’s important to scan the front side of the CNS breakdown document, not the back.

Avoid writing on the CNS breakdown to make it easier to automatically analyse the documents.

Make copies of your documents.
The mymedicis app does not store any documents. If you want to keep track of your documents, take photos of them with your smartphone and select them from the gallery when you make your claim.

Activate the “Benefit statement CNS” option only for the CNS breakdown
To simplify and speed up the processing of your claims, we recommend that you send your documents in the following order:

CNS breakdown

Invoice relating to the breakdown

Proof of payment

Pharmacy or other receipts
(invoice for lenses, doctor abroad, alternative medicine, etc.)
